 Forms can be accessed on-line at www.socialsecurity.gov. Just click the "Forms" link under "Resources", located on the right hand side of the web page.

MEDICAID ASSISTANCE (Top)

The Health and Human Service Commission is located at 12121 Westheimer.  Benefit information for:

• Medicaid for People 65 and Older and People with Disabilities
• SNAP food benefits (used to be called food stamps)
• Online application

ELECTRIC BILL ASSISTANCE (Top)

Most electric companies have offers for low income and needy customers.  If you find yourself in a situation unable to handle your utility bill there’s a program called “Lite-Up Texas.”  It is a statewide program that offers 20 percent off of your summer electric bills for hardship cases.  Many are automatically enrolled, but you can self-enroll.  You can qualify if you receive certain benefits from the Health and Human Service Commission including food stamps or Medicaid or on a low income for instance $27,563 annually for a family of four.  Apply calling 866.454.8387.
